For the Ecclesiastical Authority of the Diocese of Pennsylvania:
I hereby certify that the Rev. B. J. Maker, who has [?] to me his desire to be transferred to the Diocese of Pa is a Presbyter of Oh. Diocese as required standing, + so far as I know + believe, he’s not been justly [?] for three years last past, to end report for com[mmunity] religion or [?] of life.
