Front: Tan paper initially folded in half. Purple stamp in lower middle saying that posession of the card does not equate to Palestinian citizenship with back printed text beneath. Right side has titles printed in black, the backs of two staples, and a rectangular hand stamp towards the bottom from the Jerusalem District.
Back: Printed title and questions in black, with information written in with black cursive. Left side has a black and white photograph of a woman with dark hair pulled back and wearing plaid blouse, with a rectangular purple stamp over it. Beneath is a signature in Hebrew.
